Community Action Program - CAP Office
Our mission is to broaden the skills, knowledge and motivation within the community by identifying opportunities in employment, education, job training and adequate living conditions. This mission compels the CAP to encourage the support of the community of Tolleson by empowering families and individuals to attain self-sufficiency with the right to live in dignity. We will take pride in serving our clients with respect and a friendly attitude and not limit ourselves to the customary approaches in meeting the needs of our clients.

Diabetes Self-Management Education (DSME)
- Understanding the human body (broad overview)
- Understanding risk factors for diabetes
- Monitoring one’s body
- Increasing physical activity
- Managing diabetes through meal planning (highly interactive session)
- Learning about medications
- Living with diabetes

|AHCCCS||This link will take you to the Arizona Health Care Cost Containment System (AHCCCS) is Arizona's Medicaid agency website. They offer health care programs to serve Arizona residents. Individuals must meet certain income and other requirements to obtain services.|

They offer assistance in areas such as; adult day health care, back to school, basic needs referrals, career seeking services, case management SAIL, clothing resources, discount programs, employers services, employment resources in your neighborhood, energy/utility assistance, food resources, holiday programs, home delivered meals, home nursing, homeless assistance, homemaking services, job network club, mortgage assistance, personal care/home health aid, rental assistance, resume rescue lab, secrets to successful job searching and veteran services.|
